FAQ
Is this the same as a liability waiver?
No. A liability waiver is a separate legal release some churches also use. The parent information sheet is a safety and contact document — who this child is, who to call, and what nursery staff need to know before something goes wrong, not a release of liability.
Do we need a new sheet every single week?
Most churches use one sheet per child per family and update it every few months, or whenever contact info, allergies, or the pickup list changes — then use the shorter sign-in/sign-out log for the weekly attendance record. First-time visitors should always fill one out before their child is left in the nursery.
What if a parent won't share medical information?
Make the allergy and medical fields required before drop-off, not optional — most children's ministry insurance and safety policies expect it. If a family declines, note that on the sheet and have a staff member keep the parent's phone on hand instead.
Is the free template enough, or do we need the paid kit?
