The free generator and templates on this site are free to use, print, edit and adapt for your own church nursery, and free to share with other churches. You may not republish them as your own template library or resell them.

Paid kit. The Church Nursery Forms Kit is a digital download sold through Gumroad. Because it is delivered instantly, refunds are handled under Gumroad's refund policy — if it isn't what you expected, contact us and we will approve the refund.

No warranty. The forms are provided as-is and are not legal advice. Follow your own church's safe-church policy, your denomination's guidance, and your insurer's requirements for nursery safety, medical information and child pickup.
