Free printable church nursery forms
The parent information sheet answers who a child is. This page covers the rest of what a nursery check-in table runs on every Sunday — starting with a free sign-in/sign-out log.
When to use it
Most nurseries need at least three things on a normal Sunday: an information sheet per child, a sign-in/sign-out log for the room, and a way to flag an allergy at a glance. Searching for each separately usually turns up a single scanned form, not a matching set.
Old scanned forms are often missing fields modern check-in systems expect — a security tag number, a specific medication time, a second emergency contact — and nobody wants to be the one to redesign them from a blank page.
The free templates here work with or without check-in software — print them as a manual backup even if your church already runs an electronic system.

What to put in it
- A parent/guardian information sheet, filled out once per child and updated each term.
- A sign-in/sign-out log for the room, filled out at every single service.
- A visible allergy or medical alert — something a helper glances at, not something buried in a filing binder.
- A way to record what happened if a child gets hurt or something goes wrong, separate from the daily log.
- Security tag or claim-check numbers if your church uses matching tags for drop-off and pickup.
- A staff sign-off line, so it's clear who was on duty for a given service if a question comes up later.
Mistakes that make a nursery drop-off go wrong
A weekly attendance log has no room for allergy detail, and cramming it in there means the one week a substitute worker is in the room, they won't see it. Keep allergy information on its own sheet or card.
If your church uses claim-check tags for pickup security, the log needs a tag-number column that matches the tag on the child's bag or the parent's receipt — not a description like "blue tag," which several families can end up holding at once.
A last-minute run to the church office copier is how nurseries end up back on scrap paper. Print a stack of blank sign-in logs and information sheets at the start of the month, not the morning of.
FAQ
Are these forms specific to one denomination?
No. The fields — child info, parent contact, allergies, pickup authorization, sign-in/sign-out, incident reporting — are the same regardless of denomination or church size. Adjust the room names and service times to match your own schedule.
Can we use these alongside a check-in app like Planning Center or Church Community Builder?
Yes. Most churches keep a printed backup of exactly these documents even with check-in software, for the Sundays wifi or the app itself has a problem.
Do the free templates include the allergy card and incident report?
The two free pages here cover the parent information sheet and the sign-in/sign-out log. The allergy alert card and the incident/injury report form are part of the $7 Church Nursery Forms Kit, along with print-ready PDF versions of both free templates.
Is there a cost to use the free templates?
No. Both free templates on this site are free to copy, print, and adapt for your own church's nursery, with no email or account required.
